Who refers to us.

GPs and specialists

Send current medications and care needs. Our Registered Nurse assesses at home, builds the care plan and keeps you updated. You keep clinical oversight.

Hospitals and discharge planners

Tell us the discharge date. We line up nursing visits so your patient goes home safely, and stays home. Step down care is available if home is not ready yet.

Veterans and families

Call us. For DVA Community Nursing we arrange the referral with your GP. For Support at Home or private nursing, you can refer yourself on this page.

Referral form

Two minutes, one form.

DVA accepts a contracted provider's own referral form, so this form counts as a written referral for DVA Community Nursing when an authorised referrer sends it.

What happens next.

  1. Step one

    We confirm

Within 24 to 48 hours we confirm the referral, funding and area with you.

  2. Step two

    RN assessment

Every referral starts here. Before any care begins, a Registered Nurse assesses the client at home and builds the care plan with them.

  3. Step three

    Care begins

Visits start, and the GP receives a copy of the care plan and updates on any change.

DVA Community Nursing

The referral rules, in plain English.

Taken from DVA's own guidance for community nursing providers.

  • Who can refer

A GP, a treating medical practitioner in hospital, a hospital discharge planner, or a nurse practitioner working in community nursing.

  • When a written referral is needed

When the client is new to community nursing, has been out of care for more than 28 days, is changing provider, or reaches the end of each continuous 12 months of care.

  • How long a referral lasts

Twelve months from a GP or nurse practitioner. Six weeks from a hospital, after which the GP sends a new one.

  • What format DVA accepts

The D9389 Community Nursing Referral form, the referrer's own letterhead, or a contracted provider's own referral form, like the one on this page.

  • Where it goes

Straight to the provider, not to DVA. Send it to referrals@abchs.com.au and we take it from there.

Referral questions.

Do I need the DVA D9389 form to refer a veteran?
No. DVA accepts the D9389 form, your own letterhead, or a contracted provider's own referral form. The form on this page is ours, so it counts when an authorised referrer sends it.
Who can refer a veteran for DVA Community Nursing?
A GP, a treating medical practitioner in hospital, a hospital discharge planner, or a nurse practitioner working in community nursing. Families can call us and we arrange the referral with the GP.
When does a veteran need a new referral?
When they are new to community nursing, have been out of care for more than 28 days, are changing provider, or reach 12 months of care. A hospital referral lasts six weeks, after which the GP sends a new one.
Does the veteran pay anything?
No. DVA funds Community Nursing for eligible Gold Card holders, and for White Card holders for accepted conditions. We confirm eligibility before visits start.
Is there an assessment before care starts?
Yes. Every referral, whatever the funding, starts with a Registered Nurse assessment at home. It sets the care plan and makes sure care is safe from the first visit.
How quickly will you respond?
Within 24 to 48 hours. For an urgent hospital discharge, call 02 9387 3030 so we can plan visits around the discharge date.
Can I refer for Support at Home or private nursing on this form?
Yes. Choose the funding type on the form. Support at Home needs a My Aged Care approval first. Private nursing needs no referral at all.
What information do you need?
The client's name, date of birth, suburb, funding type and DVA number if they have one, the care needed, and a discharge date if there is one. Notes or a referral letter help, but are optional.
Can a client switch to ABC Homes N Services from another provider?
Yes. For DVA Community Nursing, DVA needs a new referral when a client changes provider, so send it to us and we arrange the handover. For Support at Home, funding moves with the client and there are no exit fees.
